Team and Individual Hockey Pictures

Time:	IP's	4:30 pm
	Mites	4:30 pm
	Squirts	5:30 pm
	Peewees	5:30 pm
	Bantams	6:45 pm

(Their may be time changes due to battery recharging etc. Please be patient, we will do our best to make this time schedule happen as planned!)

• Players need to be fully dressed
• Wear light jerseys, light socks, skates and helmets off when taking picture
• IP's same except in practice jerseys
• Please be ready to go on the ice 15 minutes before picture time.
• Disks will include your son or daughter's individual picture and team picture. 
• Disks are $15 each payable to GSYHA.

Please open, print, fill out and bring with you to the picture day along with your payment.  This is our only picture day.  Please read the form for more information.

The team picture and individual pictures will be taken during your time slot.  Please have your player at the rink dressed and ready 15 minutes before the scheduled time.
